Insurance & Payment
Payment Policy
Payment or co-payment is expected at time of each appointment. If we are out of network for your insurance, we will be happy to provide a coded invoice or “superbill” that you can present to your insurer for possible reimbursement. Cash rates available upon request.
Cancellation/No Show Policy
Cancellations must be made within 24 hour of each patient’s scheduled appointment. A $75 charge will be applied to each late cancellation or no show.
In Network Patients
As a courtesy Sagebrush will perform an eligibility check and provide you with an estimate of your out of pockets costs prior to your appointment. Unfortunately, the information we receive from insurance companies is not always correct and we strongly encourage all our patients to contact their insurance company prior to their appointment to confirm their plan’s coverage details and their financial responsibility at the time of their visit.
Out of Network Patients
Out of network patients will be responsible for our self-pay rates (available upon request) and we will be happy to provide you with a coded invoice called a “superbill” you may present to your insurance for possible reimbursement.
